Fire Disaster in Northern California - Fire Victims

On July 2, 2008, Governor Arnold Schwarzenegger signed Executive Order S-08-08. Based on the recent wildfires, this order recognizes a State of Emergency to exist in the following counties: Santa Cruz, Santa Clara, Butte, Monterey, Trinity, Mendocino, Shasta, Plumas, Kern, and Mariposa. Pursuant to this Executive Order, all fees for certified copies of birth, death, fetal death, and marriage records are suspended to persons that suffered a loss of such records as a result of the wildfires. This Executive Order will remain in effect until the Governor terminates the proclaimed State of Emergency.

In addition to the Application of Vital Record, every Fire Victim is required to complete a sworn statement declaring that they are a victim of the wildfire. The Application and the Sworn Statement can be downloaded from the links below:
