How to Request a Record

THREE OPTIONS TO SUBMIT YOUR REQUEST. INSTRUCTIONS BELOW:
  1. IN-PERSON, APPOINTMENT ONLY:

  • Complete the Online Application before you come into our office or by using our public computers available in our office lobby located at 625 Court Street, Room B01, Woodland, CA 95695.
  • Make your appointment for in-person service here. Appointments available between the hours of 9:00 am to 3:00 pm (no lunch closure), Monday through Friday, excluding holidays (see 2021 calendar).
  • Provide clerk with the “Requested By” name given on your online application. 
  • Payment is collected over the counter, and the record given to you.

  

  1. BY MAIL:

  • Complete the Hardcopy Application. 
    • Authorized Certified Copies for Vital Records require the Sworn Statement MUST be signed in the presence of a Notary Public.
  • See acceptable payment methods and return completed application to: 
    Yolo County Clerk-Recorder
    PO Box 1130
    Woodland, CA 95776-1130.

 

  1. BY FAX:

  • Complete the Hardcopy Application. 
    • FBN Statements can only be filed in-person or by mail.
    • Authorized Certified Copies for Vital Records require the Sworn Statement MUST be signed in the presence of a Notary Public.
  • Send completed application via fax to (530) 666-8109.
  • Call our office at (530) 666-8130, to verify your fax has been received and provide us your credit card information for payment.
