The Yolo County Clerk-Recorder’s Office is an an agent authorized to accept passport applications.
Passport application forms and complete information on passport services and fees are available at the U.S. State Department's Passport Services and Information web site or the Yolo County Clerk/Recorder's Office.
Please Note: Many passport services are available via mail or overnight service from the National Passport Center. However, you must apply in person for a U.S. passport if any of the following descriptions apply:
- You are applying for a U.S. passport for the first time.
- You were a minor when previous U.S. passport was issued.
- Your previous U.S. passport was lost or stolen.
- Your previous U.S. passport was issued more than 15 years ago.
- You are a minor age 16 or 17. You MUST appear in person. One parent be present to sign application.
Children 15 years of age or younger must appear in person with both parents. Parents or legal guardian may apply for the child.
Both parents or legal guardian must:
- Present evidence of child's U.S. citizenship (previous passport, certified copy of birth certificate or naturalization certificate) and
- Present evidence that they are the parents or guardian (certified copy of birth certificate or court order) and
- Show valid personal identification (driver's license or passport) and
- Sign and take oath before an authorized passport acceptance agent.
IF THE SECOND PARENT IS NOT AVAILABLE TO SIGN, THE APPEARING PARENT MUST ALSO:
5. Present evidence that he/she has:
- sole legal custody of the child or
- notarized written consent (may be faxed) of the other parent for the issuance of the passport, the Consent form is available here from the U.S. State Department
- written statement explaining why non-applying parent's consent cannot be obtained.
The law requires that all applications be signed under oath under penalty of perjury.
