How to Request a Record
FOUR OPTIONS TO SUBMIT YOUR REQUEST. INSTRUCTIONS BELOW:
ONLINE (VITAL RECORD REQUESTS ONLY):
- This is our most convenient option for submitting vital requests! Skip the lines and traffic, and order from the comfort of home!
- Click HERE to visit our online vital records request platform, Vital Director. Submit the request order, complete the Vital Verify questionnaire, and pay the associated fees.
- You will receive email updates throughout the process. We review all requests within 24 hours.
IN-PERSON (VITAL & OFFICIAL RECORD REQUESTS, FBN FILINGS):
- Complete the In-Person Application Request prior to your visit or when you arrive in our office.
- Provide the clerk with the “Requested By” name given on your online application.
- Payment is collected at point of sale in the office (not online), and the record given to at the service counter.
BY MAIL (VITAL & OFFICIAL RECORD REQUESTS, FBN, NOTARY, NOD/NOE FILINGS):
- Complete the Hardcopy Application.
- Authorized Certified Copies for Vital Records require the Sworn Statement MUST be signed in the presence of a Notary Public. Please note only Authorized Individuals may receive a certified copy. Unauthorized individuals may receive Informational certified copies of birth, death, and public marriage certificates.

BY FAX (VITAL RECORDS ONLY):
- Complete the Hardcopy Application.
- FBN Statements can only be filed in-person or by mail.

- Call our office at (530) 666-8130, to verify your fax has been received and provide us your credit card information for payment.
